NASRN Adoption Questionnaire

Please take a few moments to fill out this form, and give us a little information about yourself. All information requested is helpful in matching prospective homes with the right dog, so please answer everything as fully as possible. Upon receipt of your request, we will try to get back to you as quickly as possible. but if you do not receive acknowledgement within 7-10 days, please email us to ensure that your form results have not been lost in "cyberspace."

Disclaimer
There is NO guarantee made, express or implied, that any person requesting to adopt a dog through NASRN, or posted on NASRN's Website, will automatically be approved.

All adoptions are subject to acceptance based on a review process that requires collecting information from references and visiting the applicant's home.  Decisions on placing dogs in adoptive and foster homes are based solely on the judgment of NASRN's volunteers and coordinators, and adoption applications may be denied for various reasons.

NASRN reserves the right to refuse to adopt to anyone without disclosing the reasons, and to make all decisions regarding placement or final disposition of any rescued dog in its care whether in, or being placed in, a foster or adoptive home or other facility.

Please Note: Our rescues will not be available to working LGD homes unless the dog has been tested and evaluated with livestock and has proven to be trustworthy with them. We cannot adopt a dog out for a specific job unless we know the dog is able to do that particular job.

Our adoption policies are as follows:

• All adoptions require secure fencing, "invisible" fencing is discouraged.
• We require a home visit before any application can be approved.
• We require a signed adoption contract, and payment of adoption fee prior to placement.
• Fee is non-refundable.
• We require that the dog be returned to our care, in the event the placement does not work out. We will take any of our rescues back, regardless of the reason.  We only ask that you give us 2 weeks notice, in order for us to find foster care for the dog.
• We do not adopt out intact dogs--all of our rescues are spayed or neutered prior to placement.
• All of our adoptions are under a 30 day probationary period. Restrictions apply, please refer to the adoption contract.
• We reserve the right to refuse any applicant.

 

In addition, some dogs posted on or through NASRN's Website are under the control of other shelters or rescue groups, and NASRN has no control over how these dogs are placed.
